Please select a form below to nominate an individual or team and submit it no later than July 1, 2023 for consideration for the 2024 ballot. Forms received after the deadline will be considered for the 2025 ballot. Please provide as much supporting documentation as you can, including statistics, achievements, supporting articles, photos, etc.
Remember, someone can be on the ballot for several years before getting voted in. We have a large pool of very qualified candidates.
